For most patients prescribed gabapentin as part of seizure management, the medication works more or less as intended — dampening abnormal electrical activity in the brain and reducing the frequency of episodes. But for a smaller subset of patients, something unexpected occurs: seizures become more frequent, more severe, or harder to control after gabapentin is introduced or after a dose change. This counterintuitive phenomenon is sometimes referred to as a paradoxical drug reaction, and it is more documented in the medical literature than many patients realize.

How Gabapentin Is Supposed to Work

As an add-on treatment, it is typically prescribed alongside other antiepileptic medications rather than as a standalone therapy.

The Paradox: When Seizure Control Deteriorates

Paradoxical reactions to antiepileptic drugs are not unique to gabapentin. Several other AEDs, including carbamazepine and vigabatrin, have documented histories of worsening specific seizure types even while controlling others. With gabapentin, the phenomenon tends to surface in particular clinical scenarios.

Research and case reports suggest that gabapentin may exacerbate seizure activity in patients with certain generalized epilepsy syndromes — particularly absence seizures and myoclonic seizures. These seizure types involve different neurological pathways than partial seizures, and a medication calibrated for one type of abnormal activity can inadvertently destabilize another.

In patients with juvenile myoclonic epilepsy (JME), for example, there are documented reports of gabapentin worsening myoclonic jerks and absence episodes even when the drug was initially prescribed with good intentions. This is a clinically significant concern because JME is not always immediately identifiable, and patients may be misclassified before a more precise diagnosis is established.

One reason paradoxical gabapentin reactions can go unrecognized for longer than they should is that seizure worsening has many potential explanations. A prescriber may initially attribute increased seizure activity to disease progression, missed doses, poor sleep, or other lifestyle factors before considering the medication itself as a contributing cause.

This is not negligence — it reflects the genuine complexity of epilepsy management. However, it does mean that patients who suspect a medication-related worsening need to communicate that suspicion clearly and persistently. Bringing a seizure log to appointments, noting the timeline of medication changes alongside seizure changes, and specifically asking your provider whether gabapentin could be a contributing factor are all reasonable and appropriate steps.
