What many patients do not fully appreciate is that this particular pairing carries a level of risk that has prompted explicit warnings from federal health agencies and major medical organizations.

How Each Drug Affects the Central Nervous System

When these two drugs are taken together, their CNS-depressant effects do not simply add together. Research suggests they may interact synergistically, meaning the combined impact on breathing and neurological function can be disproportionately greater than what either drug would produce independently.

The concern over this drug combination is not theoretical. A substantial body of evidence has accumulated linking co-prescription of gabapentinoids (which includes both gabapentin and pregabalin) with opioids to increased rates of overdose and overdose-related death.

A large population-based study published in the British Medical Journal found that patients who received gabapentin alongside opioids had a significantly higher risk of opioid-related death compared to those taking opioids alone. Studies from the United States have produced similar findings, particularly among patients with chronic pain conditions who are on long-term opioid therapy.

In response to this evidence, the U.S. Food and Drug Administration (FDA) strengthened its warnings. Gabapentin's prescribing information now includes language cautioning about the risk of respiratory depression when the drug is used with opioids, benzodiazepines, or other CNS depressants. The FDA has also required updated boxed warnings — the most serious type of warning in drug labeling — on opioid medications referencing risks associated with concurrent use of CNS depressants, including gabapentinoids.

Why This Combination Still Gets Prescribed

Given these risks, patients may reasonably wonder why physicians continue to prescribe both medications simultaneously. The answer lies in the complexity of pain management.

Some conditions involve both nociceptive pain (the type opioids address well) and neuropathic pain (which gabapentin targets more effectively). A patient recovering from spinal surgery, for instance, may experience both types simultaneously. In such cases, a prescriber may determine that the clinical benefit of combining therapies outweighs the risk — particularly when doses are carefully managed, the patient is closely monitored, and other options have been exhausted.

The key word in that equation is carefully. The danger arises when this combination is prescribed without adequate monitoring, when doses are escalated without reassessment, or when patients are not fully informed of the warning signs that require immediate attention.
